HEMA Alliance Insurance Waivers

Just a quick housekeeping item. Remember that if you belong to an insured affiliate club, there should be a signed waiver in the possession of your club/school for every insured participant.

The waivers (there are two that need to be signed each person) can be found at: http://hemaalliance.com/?page_id=1019

If you are unsure of whether your club is insured or is simply a non-insured affiliate, check with your club representative or a member of the alliance's Governing Council.

Does the "received by" need to be filled out or signed by anyone in particular (or at all)?
